Currently Dr. Rollins does not accept any form of health insurance. Fees are due at time of the
appointment via cash, check, credit card, HSA card, or Venmo.

The fee for a Comprehensive Psychological Evaluation/Fitness to Practice Evaluation is
$2,000.00 with a $250.00 deposit due at time of scheduling
The fee for a Comprehensive Mental Health Evaluation for teen/adult (not Board
requested) is $1,500.00 with a $150.00 deposit due at time of scheduling
The fee for a Comprehensive Substance Use Assessment is $1,000.00 with a $150.00 deposit due at time of scheduling
The fee for a Relapse Prevention/Recovery Evaluation is $500.00
