Dr. Rollins is a board-approved evaluator for the Arizona State Board of Nursing to conduct Comprehensive Psychological Evaluations, as well as Substance Use Assessments/Relapse & Recovery Evaluations.

Dr. Rollins conducts psychological evaluations for various health regulatory board in Arizona, such as the Arizona State Board of Nursing, and Arizona Medical Board, the Board of Education, and the State Bar of Arizona.
Psychological Evaluations for the Arizona State Board of Nursing
This assessment includes:
- Comprehensive clinical interview
- Review of the documents submitted by the Arizona State Board of Nursing
- Administration of comprehensive empirically validated psychological instruments and inventories which measure general mental health status as well as identify mental health conditions and syndromes, if applicable
- Thorough assessment of substance use disorder status (if applicable)
- Review of area of focus such as anger management, ethical considerations, and boundary violations, if applicable
- Integrated report with diagnostic considerations and treatment recommendations (if applicable)
Testing instruments include:
- Beck’s Anxiety Inventory (BAI)
- Beck’s Depression Inventory (BDI)
- Minnesota Multiphasic Personality Inventory – 2 Restructured Form (MMPI-2RF)
- PTSD Symptom Scale (PSS)
- Relapse Prevention Assessment (AWARE)
- Rotter Incomplete Sentence Blank (RISB-2)
- Substance Abuse Subtle Screening Inventory – 4 (SASSI-4)
- Drug Use Questionnaire
- AWARE relapse assessment
- Brief Resilience Form
The appointment takes approximately 4-5 hours and the integrated report will be provided approximately 1-2 weeks after the assessment appointment.

Comprehensive Substance Abuse or Relapse / Recovery Evaluations for the Arizona State Board of Nursing
This assessment includes:
- Comprehensive clinical interview
- Review of the documents submitted by the Arizona State Board of Nursing
- Administration of psychological instruments and inventories which measure levels of depression, anxiety and overall mental health status
- Comprehensive substance use and recovery history
- Integrated report with diagnostic considerations and treatment recommendations which will be given to the Arizona State Board of Nursing as well as to the nurse
- The appointment takes 2-4 hours and the integrated report will be provided in 1-2 weeks from the evaluation appointment.
Testing instruments include:
- Brief Resilience Questionnaire
- Drug Use Questionnaire
- Relapse Prevention Assessment (AWARE)
- Rotter Incomplete Sentence Blank (RISB-2)
- Substance Abuse Subtle Screening Inventory – 4 (SASSI-4)
- DSM-5 specific SUD criteria, if applicable
The appointment takes 2-3 hours and the integrated report will be provided in 1-2 weeks from the evaluation appointment.
